In order to dedicate the time of your appointment entirely to the subject of your query, and to avoid administrative delays, we have at your disposal the package with the required forms in your first appointment, in two formats:
Online / Online / Internet: If you prefer to fill out your forms before arriving for your appointment, we will send you your code via e-mail so that you can access the forms, print them, fill them out and bring them complete to your first appointment.
Directly in our office: If you prefer to avoid having to download and print the forms and prefer to fill out your forms in our office directly, please arrive 20 minutes before the time of your appointment, to allow enough time to fill out the forms .
- If you want us to coordinate the obtaining of your information, obtaining resources or care with other providers (for example your doctor, your psychiatrist, a previous therapist, etc.), once you receive your code, please download and fill out the Authorization form to exchange information with the suppliers of your interest.
